Are you wearing soft contact lenses?

Soft contact lenses are comfortable but are usually unable to correct the irregularities of the keratoconic cornea. Soft lenses drape over the irregular corneal surface causing the front surface of the lens to assume the same irregular shape as the cornea. This abnormal shape is the reason for blurred and distorted vision. As keratoconus advances, soft lenses become less effective, leaving rigid gas permeable contact lenses as the only alternative for most keratoconus patients.

Do you wear rigid gas permeable lenses?

RGP lenses are made from a rigid material that does not drape over the irregularly shaped cornea, but rather, holds its shape and creates a layer of tears between the cornea and the back surface of the lens. This tear layer neutralizes the irregular shape of the cornea. Consequently, RGPs provide better vision than soft lenses.

Unfortunately, for those with keratoconus, rigid gas permeable lenses may not provide enough vision correction. This is because RGP lenses tend to gravitate toward the steepest portion of the eye, which is usually not on the visual axis, or the central point for image focus. For this reason, keratoconus patients who choose to wear RGP lenses typically experience problems with inconsistent vision. Furthermore, rigid lenses are often uncomfortable, inconvenient, allow debris to be trapped under the lens, and may dislodge during activity.

Are you wearing “piggyback” contact lenses?

Often, eyecare practitioners will “piggyback” a rigid gas permeable lens over a soft lens to provide improved comfort and increased wear time. This approach may be more successful than soft or rigid gas permeable lenses alone, but because of the complexities of handling and care, possible eye health issues due to the oxygen permeability of the lenses, and centration difficulties, success rates are marginal.
